Doctor’s Sick Note for more than 7 days

If you have been ill for more than 7 days you may need a doctor’s certificate for employment purposes. These certificates are called ‘Fitness To Work’ Certificates.

The doctor may be able to assess your situation in a telephone appointment but usually you will need a face to face appointment.

If you have already been seen by your Doctor about this illness your certificate will be sent to you within a week.

If you have been seen at the hospital, either planned or as an emergency, the hospital doctor is responsible for issuing your certificate and you will need to contact the hospital directly.

Please allow 2 working days for your sick note to be processed. You may wish to contact the surgery first to check if the sick note has been processed.
